Forms page on OP&F website

When a new member of OP&F begins employment, any initial forms needed are usually supplied to the member by their new employer. However, as time goes on the member may need to visit the OP&F website to find whatever form they may need.

The OP&F forms page is categorized and easy to navigate and many forms have automatic fillable fields. Some forms can be filled out online, downloaded, then mailed or emailed to OP&F.
