Once all information is completed, print the form and mail or fax to OP&F. Please be sure to sign and date the form, and, if required, have it notarized.

Please complete this form to authorize your monthly benefit payment from OP&F to be electronically Direct Deposited to your designated bank account on the first business day of every month.

Once enrolled in Direct Deposit, you will receive a monthly statement from OP&F indicating the exact amount of your benefit payment and details on any deductions or reimbursements.

Where should this form be returned?

You must return this completed form with a voided check or your account's deposit slip from this account to OP&F.
