I’m turning 65 and my spouse won’t be on Medicare for a few more years. Is there anything I need to do?

It is extremely important to enroll in Medicare when you are first eligible. Both UHC and AARP will be sending the Medicare eligible member or spouse a packet 2-3 months before you he/she turns 65. The UHC packet will request a copy of your Medicare card and for you to return the completed Medicare Part B Reimbursement Form. Benefit Recipients will be reimbursed in their pension check for the basic Medicare Part B premium that they are paying once these items are received. No retroactive reimbursements are given.

If you are Medicare A&B eligible, your United HealthCare under 65 coverage will term the month before you turn 65 and in order to continue with the OP&F sponsored coverage, you will be required to complete the AARP application for the AARP Medicare Supplement plans B, F, or L. The non-Medicare spouse will continue to have coverage through the non-Medicare UHC plan. The new AARP member will receive a new ID card to use.

Must I continue to pay my AARP membership fee in order to retain my OP&F subsidized AARP coverage?

No. Once you receive your AARP ID cards and Certificate of Coverage, you may cancel your AARP membership and still remain covered. For new members to AARP, the first years membership will be paid for you by United Healthcare.

Can I enroll in Medicare Part D and still stay enrolled in the OP&F sponsored prescription drug coverage through United HealthCare?

No. You will need to decide which prescription drug plan will best fit your personal circumstances. If OP&F coverage is termed, you will have limited re-enrollment opportunities as spelled out in the Member’s Guide to Health Care Coverage.

If I have an update to my personal information such as divorce or marriage, who do I contact, UHC or OP&F?

You should send your change in writing to UHC, who will then forward the information to OP&F if applicable. If there is a discrepancy in the information that OP&F and UHC have, contact OP&F customer service for resolution.

If I need to update my address, who do I contact?

You should send your change of address in writing, with your signature, to OP&F. OP&F will update their records as well as forward the information to UHC to change their records.

Since there isn’t a vision ID card issued, how do I use my benefits?

1. Locate a provider by calling 1.800/839/3242 or by logging on to www.myuhcvision.com.

2. Call provider for appointment. Identify yourself as having United Healthcare Vision and provide the primary insured’s identification number.

3. If prescription eyewear is necessary, your UHC provider will assist with your options/selection and order your prescription.

4. If you choose to use an out-of-network provider, you will need to submit the itemized paid receipt, along with the primary ID number, patient’s name and date of birth to UHC and you will receive reimbursement for your out of network benefits.

I want to add dental and vision coverage for myself and family, how is this done?

Effective June 1, 2009, you may enroll in dental and vision for yourself and family per qualifying event and as long as you and your eligible dependents complete a Health Care Eligibility and Enrollment Form and any other required documents within 60 days of the qualifying event. You may also enroll during the Annual Change Period each year.

If the retiree/surviving spouse is not enrolled in dental and/or vision, are other family members able to enroll?

No, the retiree/surviving spouse must enroll to allow other eligible dependents to enroll.

Should I be enrolled in UHC or AARP medical coverage?

UHC coverage is for all eligible non-Medicare benefit recipients and dependents, early Medicare recipients, Medicare A only recipients, Medicare B only recipients, or OP&F retirees residing outside of the United States. AARP is for all eligible benefit recipients and dependents that are age 65 and over enrolled in Medicare A & B.

My child is turning 18 and going to college. Can she/he be covered?

Your child can continue to be covered as long as they are a full-time student at an accredited school. A Student Eligibility Form must be completed and returned to UHC for each quarter or semester that the student attends school. He/she can be enrolled until the age of 23 as long as they are a full-time student and provide the eligibility form. It is the member’s responsibility to notify UHC if a child is no longer eligible for coverage to prevent the repaying of unsubsidized monthly premiums during the period of ineligibility.

I am getting married and want to add my new spouse to coverage. What do I need to do?

You can enroll your new spouse if the following documentation is received within 60 days of the marriage

• Copy of her birth certificate
• Copy of spouse social security card
• Copy of solemnized marriage certificate with marriage date & proper authority
• Health Care Eligibility Enrollment Form

At the same time, you can enroll your new stepchildren within 60 days of the marriage, with the following documentation.

• Copy of each child’s birth certificate
• Copy of each child’s social security card
• Proof that coverage is not available through either natural parent

Where do I return my Medicare B Reimbursement Statement?

This completed form, along with a copy of your Medicare card should be sent to:

No retroactive reimbursements will be given.

I have Medicare as my primary coverage, how do I file a claim?

Automatic Claim Filing (ACF) is a process where your Medicare carrier forwards your Part B claims to UnitedHealthcare Insurance Company.

1. Medicare sends you an Explanation of Medicare Benefits (EOMB), showing their Part B eligible expenses. A message at the bottom of your EOMB tells you that your claim information was forwarded to your insurer. If there is no message on your EOMB, mail your claim to UnitedHealthcare at the address below or call the toll free number below to see if your claim has been submitted by your provider.

2. UnitedHealthcare will send you a separate Explanation of Benefits to explain what action was taken on your claim.
