In May, active OP&F members can expect to see an annual Statement of Account in their mailboxes, detailing a personalized report of total contributions paid to OP&F. The document also includes important information concerning benefits offered by OP&F.
Specific information to be included will be the taxed contributions, tax�deferred contributions, and total accumulated contributions as of Dec. 31, 2004. The Statement also includes formulas to calculate service or disability retirement.
All members are asked to verify their home mailing address, Social Security number and date of birth. All of this information is printed on the Statement of Account, along with directions on how to correct any errors.
