Personalized forms and instructions for members eligible for the OP&F�sponsored health care plan are scheduled to be mailed Sept. 29, signaling the beginning of the 2007 Annual Change Period.
OP&F continues to sponsor health care benefits for eligible benefit recipients and their dependents according to the new plan design and enrollment guidelines, which will be implemented Jan. 1, 2007. Adoption of this plan came as a result of a vote by the OP&F Board of Trustees to reaffirm their commitment to providing current and future retirees with quality health care and to develop a plan that would ensure the availability of some form of health care subsidy.
During the 2007 Annual Change Period, which runs through Oct. 31, 2006, eligible members will have the opportunity to choose the carriers best suited for them and their family. The Annual Change Period Form (ACPF) includes personalized information and the member�s current coverage. Along with the form, the Annual Change Period Guide provides more details about the 2007 OP&F�sponsored health care plan, and explains the Annual Change Period process, and offers instructions on completing the ACPF.
As in past years, a series of Annual Change Period seminars will be held around Ohio to answer questions and assist members in completing the ACPF. After receiving this information, if you have any questions regarding the Annual Change Period, please contact OP&F Customer Service at (800) 860�9599, Mon.�Fri., 8 a.m.�4:30 p.m. EST.
There are a few changes to the OP&F�sponsored health care plan that may affect you in 2007. Some of the changes include:
Again this year, medical and prescription drug coverage are offered as separate options with separate monthly contribution amounts. The ACPF can also be used to enroll eligible members or dependents in the supplemental dental and/or vision plans.
If you are currently enrolled in a PPO health care carrier under the OP&F�sponsored health care plan and do not return the ACPF by Oct. 31, 2006, you will automatically be re�enrolled with the same medical and prescription drug carriers. If you are currently enrolled in an HMO plan, you will automatically be assigned to the Aetna PPO or Medical Mutual PPO. If you do not want to enroll in the carrier assigned to you, you must make the correction on your ACPF and return it to OP&F by Oct. 31, 2006.
